Headache, menstrual cramps, back pain, or joint pain — we all feel pain differently. In Ghana, you’ll find many options like Paracetamol, Ibuprofen, Diclofenac, and others. But “strongest” doesn’t always mean “best.”

Step 1: Understand the Main Types
1. Paracetamol (Acetaminophen)
Best for: Headache, fever, toothache, mild pain, menstrual cramps.
Why people choose it: Gentle on the stomach. Safe for most adults, pregnant women, and children when dosed correctly.
Watch out: Overdose can damage the liver. Don’t exceed 4g per day for adults. Avoid alcohol excess.
2. NSAIDs – Non-Steroidal Anti-Inflammatory Drugs
Examples: Ibuprofen (Brufen), Diclofenac (Voltaren), Aspirin, Naproxen.
Best for: Pain WITH swelling/inflammation — arthritis, sprains, back pain, menstrual pain, dental pain.
Why people choose it: Reduces both pain and inflammation.
Watch out: Can irritate the stomach, raise blood pressure, and affect kidneys. Take after food, with water. Not ideal if you have ulcer, asthma, hypertension, kidney disease, or if pregnant.
3. Topical Pain Relievers
Examples: Diclofenac gel, Menthol patches/balms (Deep Heat), Capsaicin cream.
Best for: Muscle pain, knee pain, neck pain, sprains.
Why people choose it: Targets pain directly, fewer whole-body side effects.
Watch out: For external use only. Don’t apply on broken skin.
4. Combination Medicines
Examples: Paracetamol + Diclofenac, Paracetamol + Ibuprofen, Paracetamol + Caffeine.
Best for: When single medicine is not enough.
Watch out: Many contain paracetamol hidden inside — you can easily double-dose. Always check the label.
Step 2: Choose Based on Your Pain
1. Pain Type: Headache / Fever
Usually Recommended First: Paracetamol
Why: Safe, effective, gentle on stomach
2. Pain Type: Toothache / Menstrual cramps
Usually Recommended First: Ibuprofen or Diclofenac after food
Why: Reduces inflammation that causes pain
3. Pain Type: Muscle / Joint / Back Pain
Usually Recommended First: Topical gel + Paracetamol or NSAID
Why: Combination gives fast + lasting relief
4. Pain Type: Arthritis in older adults
Usually Recommended First: Paracetamol first, topical NSAID second
Why: Lower risk of stomach/kidney issues
Step 3: Check Your Personal Safety
Before you buy any painkiller in Ghana, ask yourself:
1. Do you have an ulcer, hypertension, asthma, kidney or liver disease?
If yes, avoid self-medicating with NSAIDs. Talk to a Citi Pharmacy pharmacist first.
2. Are you pregnant, breastfeeding, or giving to a child?
Paracetamol is generally first-line in pregnancy. NSAIDs like ibuprofen and diclofenac are NOT recommended in late pregnancy. Aspirin is not for children under 16.
3. Are you taking other medicines?
NSAIDs + blood pressure drugs or blood thinners = risky. Paracetamol + alcohol = liver risk.
4. How long has the pain lasted?
If pain lasts >3 days, is severe, comes with swelling, fever, vomiting, or you need painkillers daily, see a doctor. Don’t just increase the dose.

For most adults without liver disease, paracetamol taken at the correct dose is considered the safest for headache, fever, and mild pain. It has fewer stomach and blood pressure effects than NSAIDs.
